Disease management programs earn award for outcomes

HealthPartners is getting some special attention for its disease management success. The Disease Management Purchasing Consortium has awarded HealthPartners the Excellence in Disease Management Outcomes award.

The award recognizes HealthPartners for having the highest validly verified return-on-investment (ROI) in disease management of any health plan in Minnesota.   The ROI is calculated from data that focused on patients with five common chronic conditions from 2001 to 2009. During that period, the adverse medical event rates for HealthPartners plan members in those groups fell by more than 35 percent, the best performance of any health plan in the Disease Management Purchasing Consortium database.  

For instance, if the ratio had stayed the same in 2009, HealthPartners members would have had 1,400 more heart attacks and other blood-constriction events (such as angina), asthma attacks, out-of-control glycemic episodes, fluid overload and shortness of breath incidents. More incidents lead to increased medical expenses and cause lost work time, not to mention the additional burden on those patients and their families.
