Applying for an individual plan with HealthPartners is simple. Here is your step-by-step guide to the process so you know what to expect.
Step 1: Determine Your Annual Medical Costs. HealthPartners knows that you have a lot of choices, and the options can sometimes be confusing. We have a number of online tools to help you make decisions, but a good place to start is determining your annual medical costs. You can also contact us and talk to a HealthPartners sales representative who can answer your questions and discuss your options.
Step 3: Complete Your Application. Once you know which plan and deductible you want to apply for, complete an application online or via fax or mail.
A payment for your first month's premium is due with your application. We will not process the payment until you have been approved for the plan you selected.
If you apply for a Short Term plan, you must submit with your application a payment for your total premium and a $20 application fee.

Step 4: Understand the Review Process. Once we have received your completed and signed application and your first month's premium payment, we send it to our underwriting team (the short term plan does not require underwriting).
Upon receipt of your enrollment form, we will review it for completeness. We may need to contact you for further details or we may need to request health history information from other healthcare providers. This is a normal part of the process, and we will notify you of any such request. Please note that you may be billed by your health care provider for the necessary records.
We will notify you of a decision after your enrollment form and any additional information have been reviewed. Normal processing time is one to two weeks. A decision may take longer if additional medical information is needed. The faster our underwriters get your documents, the faster they are able to make a decision. Your application can be reviewed quickly when you work closely with your underwriter, doctor and clinic.
If you are approved for the HealthPartners plan you selected, you will be automatically enrolled in that plan for the next available effective date (either the 1st or 16th of a month) and the first month's premium payment you submitted with your enrollment form will be processed.
If you are not approved for the HealthPartners plan you selected on your application, we will notify you of the reason(s) for the decision and provide you with information on other options.
What happens if I am not approved? You still have options even if you're not approved by HealthPartners for individual coverage. HealthPartners is happy to offer you the Minnesota Comprehensive Health Association's insurance, which is administered by a private company. This product is guaranteed issue. What if I cannot afford the premium? Minnesota has a variety of public programs designed to assist low-income residents with health care. Unlike private individual insurance through HealthPartners, participation in state programs requires continued verification of eligibility.
