To make a referral, call 952-883-6875 with the following information: - Demographic information
- Physician order for home care
- Home care diagnosis
- Current medications
- Medical history and other relevant information
An initial evaluation visit generally occurs within 48 hours after receiving your referral. A nurse or therapist will meet with you in the privacy of your home to discuss your needs. For more information call 952-883-6875.
