Minnesota Health Care Programs
HealthPartners Care plans provide high quality care and services for participants receiving medical assistance through one of the Minnesota Health Care Programs. HealthPartners contracts with the Minnesota Department of Human Services to offer the following plans:

HealthPartners Care® PMAP is primarily a federally-funded plan for low-income families with children and seniors.

HealthPartners Care® PGAMC is a state-funded plan for low-income adults who have no dependent children.

HealthPartners Care® MinnesotaCare is a state-funded plan for residents who do not have access to affordable health care coverage.

HealthPartners® Classic Minnesota Senior Health Options (MSHO) Plan is specifically designed for people over age 65 that are eligible for Medical Assistance with or without Medicare Parts A and B. This plan offers easy access to primary and specialty health care with the one benefit that is most comforting: your own Care Coordinator to help coordinate your care.

HealthPartners Minnesota Senior Care (MSC) is a state-funded plan for low-income seniors.
