2008 HealthPartners Care Blood Lead Incentive Program
Making sure our members are healthy is important to HealthPartners. Getting a blood lead test done around age one and two is an important part of making sure children are healthy. A blood lead test is also a component of a complete C&TC for certain age groups.
9-30 Month Blood Lead Incentive
HealthPartners offers a $25.00 gift card incentive to HealthPartners Care members that are 9-30 months that receive a blood lead test. All HealthPartners Care members that fit this age group and receive a blood lead test are eligible for the incentive. Members can print this voucher and bring to the clinic. A physician or lab signature is required.
Targeted Mailing - Blood Lead Incentive
HealthPartners will also do direct incentive brochure mailings to HealthPartners Care members that are 18-30 months. A $25.00 gift card incentive will be sent to eligible members that receive a blood lead test. A physician or lab signature is required.
C&TC Incentive Program
The C&TC incentive is offered to HealthPartners Care members ages 1-5. Eligible members receive a letter about the program. The letter has a voucher for the provider to sign when the exam is done. After the member returns the signed voucher to HealthPartners in the postage paid envelope, a $25 Target gift card is mailed to the member.
If the child is due for a blood lead test, they can also receive another $25 gift card for having that test done. The member will need to send the blood lead voucher to HealthPartners in order to receive the gift card.
One gift card per incentive per member. Gift cards will be sent within 4-5 weeks of receipt of signed voucher.
