As a HealthPartners nurse you will be part of a collaborative team, playing an integral role in the care coordination of our patients. Nurses link their efforts with other disciplines and resources across the continuum of care for the benefit of our patients.
At HealthPartners clinics we are focusing on redesigning and improving access to care for our patients. We are building a care system that streamlines our care processes. Our focus on continuous care improvement is opening up some exciting new challenges for our nursing staff, who are finding more and more opportunities for teaching, coordinating care, increasing patient/family self-management skills and proactively monitoring patients to prevent problems and enhance quality of life. Our patients are at the heart of everything we do at HealthPartners.
