2008 HealthPartners Classic Plan
The HealthPartners® Classic plan is a Medicare Advantage plan available to Medicare beneficiaries within the HealthPartners® Classic Plan Service Area.
The HealthPartners® Classic plan covers all the benefits you have with Medicare, plus benefits that Medicare does not cover.
With HealthPartners® Classic, you pay no copayments for preventive and most health services and you pay less for many services than you do under Original Medicare.
Plus, HealthPartners handles most of your Medicare paperwork and coordinates your benefits. You must receive your care from the HealthPartners Classic Plan network, except for emergency and urgent care, renal dialysis or as provided under the travel benefit.

How to Enroll
Please read the plan's summary of benefits prior to enrolling. Complete the
HealthPartners® Classic Individual Enrollment Form
and mail it to HealthPartners.
Travel Coverage
An added bonus is a travel benefit that covers members while traveling outside of Minnesota. You can receive services from non-contracted providers while vacationing or temporarily outside of Minnesota for up to six months. That way, you can plan your preventive care visit before you leave Minnesota and not have to worry if you'll need care while traveling.
