- Our Frequent Fitness program works with health clubs and other community partners to offer incentives for our members to improve their health.
- Our innovative health plans are designed to reward members and caregivers for choosing and delivering the best care.
- We measure our success and we know that our disease management programs have dramatically improved the lives of patients with chronic conditions including diabetes, heart disease
National grant
We are recognized nationally for our outstanding service and care. HealthPartners is one of only seven health care organizations in the nation to receive a grant from the Robert Wood Johnson Foundation to transform our health care system based on six aims: to provide care that is patient-centered, safe, timely, equitable, efficient and effective.
