Applying for coverage with HealthPartners is easy
Your part of the application process is simple. Here is your step-by-step guide so you know what to expect.
Step 1: Select the plan and deductible you want.
HealthPartners knows that you have a lot of choices, and the options can sometimes be confusing. We have a simple interactive tool that you can use to find a plan that works best for you and your family. You can also contact one of our friendly representatives to ask questions and get help with your decision.
Step 2: Complete Your Application.
Once you've selected a plan and deductible, get a quote and apply online, or you can complete a paper enrollment form and mail or fax it to us.
Comprehensive Medical Plans
A payment for your first month's premium is due with your application for a Traditional, Empower or Three for Free individual plan. We will not process the payment until you have been approved for the plan you selected.
Once you've been approved for the plan of your choice, you can choose your preferred method of ongoing payment. Options include automatic monthly withdrawal from a checking or savings account or quarterly (four times per year) billing.
Short Term Medical Plans
Payment for the total premium and a $20 application fee are due with your application for a Short Term plan.
Personal Dental Plans
Payment for the first months premium is due with your application for a dental plan. Please also select your preferred method of payment on your application.
Step 3: Understand the Review Process.
If you apply for a comprehensive plan:
Once we have received your completed and signed application and your first month's premium payment, we send it to our underwriting team.
We may need to contact you for further details or we may need to request health history information from other healthcare providers. This is a normal part of the process, and we will notify you of any such request. Please note that you may be billed by your health care provider for the necessary records.
We will notify you of a decision after your enrollment form and any additional information have been reviewed. Normal processing time is one to two weeks. A decision may take longer if additional medical information is needed. The faster our underwriters get your documents, the faster they are able to make a decision. Your application can be reviewed quickly when you work closely with your underwriter, doctor and clinic.
If you are approved for the HealthPartners plan you selected, you will be automatically enrolled in that plan for the next available effective date (either the 1st or 16th of a month) and the first month's premium payment you submitted with your enrollment form will be processed.
If you are not approved for the HealthPartners plan you selected on your application, we will notify you of the reason(s) for the decision and provide you with information on other options.
If you apply for a short term medical or dental plan:
Your coverage begins as soon as we receive your application and payment for your first months premium, if youre eligible.
What happens if I don't get an offer?
You still have options even if you're not approved by HealthPartners for individual coverage. We can provide you with information about guaranteed-issue insurance from the Minnesota Comprehensive Health Association (MCHA), which is administered by a private company. What if I cannot afford the premium?
Minnesota has a variety of public programs designed to assist low-income residents with health care. Unlike private individual insurance through HealthPartners, participation in state programs requires continued verification of eligibility. Additional information is available at dhs.state.mn.us.
