MN Community Measurement Recognizes HealthPartners Medical Group for Consistent Quality Health Care
2007 Quality Ratings measure 128 Medical Groups across Minnesota

Bloomington, Minn. – MN Community Measurement recognized the HealthPartners Medical Group for providing consistent high quality care for patients in its 2007 annual performance ratings, released yesterday. MN Community Measurement is a St. Paul-based nonprofit group dedicated to improving health care quality.
MN Community Measurement observed that the HealthPartners Medical Group scored three stars (top performance) on 10 of 12 quality measures. This is the second consecutive year that HealthPartners Medical Group has registered top performance in at least 10 of the 12 measures, which include: optimal diabetes care, controlling high blood pressure, proper asthma medication management, optimal vascular care, proper treatment of colds in children, proper testing of sore throats in children, children's immunizations, optimal cancer screening (composite), breast cancer screening, cervical cancer screening, colorectal cancer screening and Chlamydia screening.
"Our performance is a direct result of our work to redesign the way we deliver care," said Beth Averbeck, M.D. associate medical director, health care improvement for HealthPartners. In 2004, HealthPartners Medical Group began redesigning its workflows and practice model to maximize the skills of Prepared Practice Teams. These teams consist of a physician, nurse, receptionist and other staff who work together and directly with patients to provide consistent high quality care and patient experience. The teams leverage a sophisticated electronic medical record to ensure health information availability for caregivers and patients at all times.
Teams focus work around five core patient interactions including: scheduling, pre-visit planning, check-in, visit, and post visit. The model anticipates patient needs before a visit, reminds patients about important health issues and provides follow-up and ongoing support for specific patient needs. The American Medical Group Association honored HealthPartners Medical Group for its redesign work with the 2006 Acclaim Award.
