Individual (Under age 65)

Traditional – Comprehensive coverage with a wide range of deductibles to fit different budgets
Empower – A high-deductible plan to pair with a Health Savings Account, plus first dollar preventive coverage
Three for Free – Balanced, affordable coverage with three free office visits and $5 copays on generic medications
Short Term – Temporary coverage when your clients need instant protection
Closed Plans – Information and rates for the Individual Open Access and Empower HSA plans. These plans are closed to new applicants as of July 1, 2008. Current members may be able to add dependents to their contracts.

Our coverage features:- Affordable medical, prescription drug and dental plan options. Some are stand-alone and others are embedded, giving your clients a variety of options.
- Coverage for residents in Minnesota, Wisconsin, and elsewhere!
- A large provider network – and we do not require referrals!
- Travel benefits that include up to 9 mos. in the United States and worldwide coverage for emergency and urgently needed care.
- Programs to help maintain a healthy lifestyle, including fitness club discounts and other wellness programs.
Freedom – Our most flexible and popular product! We have three medical options, three drug options and two dental options. (Wisconsin residents have more restricted choices.) Your clients can choose which option is the best fit for them.
Classic – Medical and integrated prescription drug coverage. This plan also has an optional dental plan associated with it.
Liberty – This medical plan with integrated prescription drug coverage is a Private Fee-for-Service plan that features coverage outside Minnesota and Western Wisconsin.
