Freedom Plan
 The HealthPartners® Freedom plan is a Medicare Cost plan that offers your clients the flexibility to choose the medical, prescription drug and dental plans that best fit their needs and budget. Our affordable family of plans will be there now and as your clients' needs change over time.
The Freedom plan is available to Medicare-eligible residents of Minnesota and eight counties in western Wisconsin. The plan has an extensive provider network, which means your clients' doctors are most likely already part of our network. No referrals are required to see specialists. Members of the Freedom plan also have access to three different fitness club programs, as well as other healthy living and wellness programs.
Minnesota residents are eligible for three medical, three prescription drug, and two dental options. Please note: Freedom Plan I is not eligible to add drug coverage. Wisconsin residents are also eligible for two medical plans and a drug plan.

Classic Plan
The HealthPartners® Classic plan is a Medicare Advantage plan that gives your clients the peace of mind of having their medical and prescription drug benefits embedded in the same plan. An optional dental plan is also available with this plan. The Classic plan is available to Medicare-eligible people who live in the 13-county Twin Cities service area. The Classic plan also provides coordinated care between the HealthPartners clinics, hospitals and other providers available in the network. No referrals are required. Members of the Classic plan also have access to two fitness club programs, as well as other healthy living and wellness programs.

Liberty Plan
The HealthPartners® Liberty plan is a Medicare Advantage Private Fee-for-Service plan. It gives your clients the peace of mind of having their medical and prescription drug benefits embedded in the same plan, while giving them the freedom to see any provider who will accept the plan's terms and conditions. This plan is available to Medicare-eligible people in Minnesota who live outside the 13-county Twin Cities metro area and people who live in one of 23 western Wisconsin counties.
Members of the Liberty plan have access to a fitness club discount program and other healthy living and wellness programs.
Important information for your Medicare clients: The HealthPartners® Liberty plan is a Medicare Advantage Private Fee-for-Service plan that works differently than a Medicare supplement plan. Your doctor or hospital is not required to agree to accept the plan's terms and conditions, and thus may choose not to treat you, with the exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may choose not to provide health care services to you, except in emergencies. Use this Liberty Plan Checklist  with your clients to make sure they have considered the important things before enrolling in this plan. This includes checking with their providers to make sure they accept Medicare Advantage Private Fee-for-Service plans, including the Liberty plan.
