 Our unique health assessment identifies the 20% of members at risk for disease who drive 80% of health care costs. The health assessment evaluates their current health status and helps determine if they are at risk for serious health conditions like diabetes and heart disease. It can identify members at risk up to two years in advance of disease onset so we can take a proactive care approach.
We can engage these members in proven disease management and health improvement programs that help employers manage cost trend and have a healthier, more productive workforce. Studies show a reduction of $3,000-$5,000 in future claims costs for employees identified as at risk for diabetes or heart disease.*

Participants complete a 15-minute assessment with questions that focus on their health, lifestyle and health history. They'll receive points for each of their answers, which are used to calculate their health score and health potential score. The health score indicates what they are doing right and where they need to make improvements to live healthier. Their health potential score tells them just how much they could gain by making lifestyle changes.
