Health Assessment

HealthPartners Health InvestmentSM Program identifies the individuals driving cost, engages them to take charge of their health and delivers savings to you. A comprehensive campaign, that includes incentives for health assessment completion, and unique follow-up plan are the keys to the program's success.

What is Health Assessment?

HealthPartners health assessment engages participants by focusing on their health potential. While the goal of the tool is to identify individuals with or at risk for diabetes or heart disease, participants are more likely to get and stay engaged if their experience with the health assessment is positive.

Participants receive the following information, which provides them with a picture of their health, and tools to help them take the next step to address any health issues:

  • Health score - The participant's health status is translated into a score, taking into account their health history and reported health habits. Based on the most recent health and medical research, participants receive their score out of a possible 1,000 points. The score includes five high-impact areas—sleep, smoking, diet, healthy weight and physical activity—which are also factors that can be modified, and improved, by changes in lifestyle habits.
  • Detailed personal report - This report shows the small changes the participant can make that will improve their health and increase their health score.
  • Health tips - This section provides a full online library of tips and information on topics ranging from tobacco cessation to sleep, diabetes to healthy eating.
  • Personalized preventive care checklists - Based on gender, age and health condition, these checklists serve as a tool to facilitate patient-physician discussions, ensuring that participants know about and receive the health care services appropriate for them.

Follow-up

Our systematic approach to identifying an individual's health issues begins when the completed health assessment arrives at HealthPartners. You can even choose to provide the health assessment to their employees and dependents who are not HealthPartners members.

HealthPartners Members

All HealthPartners members who complete the health assessment receive the following services:

  • Unique health score - based on the most recent health and medical research, and taking into account their health history and health-related habits
  • Detailed personal report - shows those small changes that can improve the individuals health the most and increase their health score
  • Health tips - dozens of effective health improvement tips and step-by-step instructions to lose weight, manage stress, quit smoking, and more
  • Personalized checklists - based on the participant's age and gender, showing how often they should see a doctor and the tests and services they should be receiving
As part of their health assessment follow-up report, all members will receive information on programs and resources that are available to help them take the next step to improving their health. Some of these programs have fees; others are available to download through the health assessment; and others direct the individual toward community-based programs. Information on the following programs, services and resources is provided to members through the health assessment:

  • Frequent Fitness
  • 10,000 StepsSM program (Member Cost: $20)
  • Weight Watchers
  • A Call to Change Partners in QuittingSM (smoking cessation)
  • Dragon Calendar (smoking cessation)
  • Bear Calendar (weight management)
  • Information library

HealthPartners Non-members

Non-HealthPartners members who complete and submit the health assessment can receive limited services for an additional fee. Employers must decide prior to the health assessment implementation whether to offer the standard or the enhanced follow-up to their employees.
