Disclosure of Ownership


HealthPartners worked collaboratively with the Minnesota Council of Health Plans and several other Minnesota payers to create a standard form that can be used to submit information related to disclosure of ownership, business transactions and excluded providers. You need only complete this form once and distribute it to the health plans you have contracts with.

Disclosure of this information is a requirement from the Minnesota Department of Human Services (DHS) and the Centers for Medicare and Medicaid (CMS). They require all health plans, including HealthPartners, to ensure its network providers submit documentation of their ownership, business transactions, and providers excluded from participation in government funded health care.





Fraud, Waste & Abuse Training

This training requirement from CMS applies to all organizations that provide health care services or administrative services for Medicare-eligible individuals under Medicare Advantage or Part D programs. Employees who provide covered services to Medicare beneficiaries will need to complete this training by December 31, 2009. If you have provided your own fraud, waste and abuse training to your employees and it meets the CMS requirements outlined in the attached memo from CMS, please indicate that on the attestation form.

Fraud, Waste and Abuse Presentation

If your organization contracts with other entities to provide health care or administrative services on behalf of Medicare Advantage or Part D members, the training materials need to be provided by your organization. Your organization will be required to attest that your organization has obtained attestations from those entities that they have completed the fraud, waste and abuse compliance training.

Providers are responsible for tracking their organizations completion of this training.
