Capitol View Transitional Care Center provides short-term intensive therapy and nursing care following hospitalization. We tailor a rehabilitation plan to meet individual needs and get you on the road to recovery.

Home Care

Integrated Home Care is a Medicare-certified and state-licensed agency providing comprehensive services that promote healing at home. Using a customized and personalized care plan, this team helps patients and their families move safely, efficiently and comfortably from hospital or transitional care to home.

Hospice

Hospice & Palliative Care is a Medicare-certified and state-licensed care program for patients and families facing end-of-life issues. It is available to patients with a terminal diagnosis who are living in a private residence, nursing home, assisted living facility or group home.
