How to obtain Home Care services
To make a referral, call 952-883-6875 with the following information:
- Demographic information
- Physician order for home care
- Home care diagnosis
- Current medications
- Medical history and other relevant information
An initial evaluation visit generally occurs within 48 hours after receiving your referral. A nurse or therapist will meet with you in the privacy of your home to discuss your needs.

Falls prevention program
Are you or a loved one worried about falling? More seniors are injured falling in their homes than by any other accident. This program works with patients and families to identify risk factors and appropriately intervene to improve home safety. When physician-ordered, the service is covered by most major health plans. - Home evaluations
- A balance screening
- Recommendations for adaptive equipment or home modifications
