Our physicians and nurse practitioners work as teams to bring care to residents at metropolitan area nursing facilities and assisted living residences. Our medical care teams make both regular and urgent visits and coordinate all needed medical care services. We have a medical care team at most metro area nursing facilities and at many assisted living residences in the metro area.
Our goal is to keep you as healthy as possible. The physician and nurse practitioner team will involve you and your family members as they create and implement a personalized care plan, and they will communicate with you when the care plan needs significant change.
HealthPartners Geriatrics is part of the Division of Home Care, Hospice and Geriatrics in the HealthPartners Medical Group. We have been helping older adults stay healthy since 1973, and now serve over 3,000 residents at community care facilities.
