A transitional care center is a specialized unit within a health care facility that focuses on restorative and rehabilitative care. It is usually located within a community health care facility, but has its own separate space and professional staff.

Transitional care centers provide short-term care for patients who do not need the intensive services of a hospital, but are not well enough to be at home. Most patients come to recover after a hospital stay, but some may be admitted directly from their homes, clinic or emergency department.

These centers are pleasant and less hectic than a hospital. Comfortable semi-private rooms are equipped with a telephone, electric bed and TV. Meals are served in a dining area or in your room, if you prefer. Staff is present 24 hours a day and encourages you to be as independent as possible in an effort to go home.
