Three for Free plan
So what does “free” really mean? HealthPartners pays the doctor’s fees for your first three office, urgent care or convenience clinic visits. You are responsible for other services you receive during the office visit like lab tests, x-rays, MRIs, CT scans and some types of therapy. For these expenses, you are responsible for all costs up to your deductible.
After the first three visits, office visits are subject to deductible and coinsurance. If you reach your deductible, you pay a percent of all costs up to your out-of-pocket maximum. Once you reach your out-of-pocket maximum, you pay nothing.
Three for Copay plan
Three for a Copay plans are very similar to the Three for Free plan, but with a twist. You pay a $25 copay for the first three office, urgent care or convenience care clinic visits. Labs, prescriptions and X-rays are additional and subject to the deductible.
Your first three office or urgent care visits will cost you $25 each. After the first three visits, office visits are subject to deductible and coinsurance. If you need other services during your visits like lab tests, x-rays, MRI and CT scans, you are responsible for all coinsurance costs up to your deductible. If you reach your deductible, you pay a percent of all costs up to your out-of-pocket maximum. After you reach your out-of-pocket maximum, you will only have to pay for office visit or pharmacy copays.
