HealthPartners Freedom and HealthPartners Wisconsin Freedom

To join one of these plans, you must:

  • Be enrolled in Medicare Parts A and/or B; and
  • Live in the service area ; and
  • Not have end-stage renal disease (for exceptions, see the Summary of Benefits).

 This year, we’ve got more ways for you to enroll, making sign-up even easier!


Option 1: Enroll online

  • By completing an enrollment form online, you are requesting enrollment in HealthPartners Freedom. You will receive notice in the mail acknowledging receipt of your completed enrollment request or requesting additional information.
  • Medicare beneficiaries may also enroll in HealthPartners Freedom through the CMS Medicare Online Enrollment Center.

Option 2: Enroll over the phone by calling 952-883-7788 or 877-240-8311

  • You will be guided through the application process by a representative who asks you the questions on the application. Please note that this number is only for enrollment and cannot be used to answer questions about the plans or provide more information. For those questions, call Medicare Sales before calling this number.

Option 3: Download the HealthPartners Freedom Enrollment Form and mail or fax it to us.

Once you've printed and filled the form out, simply mail it to the address listed on the application.

HealthPartners Minnesota Senior Health Options (MSHO)

To join this plan, you must:

  • Be 65 years or older; and
  • Have Medical Assistance and Medicare Parts A and B; and
  • Live in Anoka, Benton, Carver, Chisago, Dakota, Hennepin, Ramsey, Scott, Sherburne, Stearns, Washington or Wright county

To enroll:

Download the HealthPartners MSHO Enrollment Form and mail or fax it to us as indicated on the application.
