Minnesota Senior Health Options

HealthPartners Minnesota Senior Health Options (MSHO) (HMO SNP) is designed for seniors on Medical Assistance and Medicare Parts A and B. MSHO offers easy access to primary and specialty health services and your own Care Coordinator to make sure you get the care you need.

Plus, the MSHO plan received Medicare’s rating of 4.5 stars in 2013. HealthPartners MSHO is one of a small number of special needs plans throughout the country and the only MSHO plan in Minnesota to achieve a 4.5 star rating in 2013. This puts HealthPartners in the top 10 percent of all Medicare Advantage plans in the nation*.

The MSHO plan pairs your medical benefits – including coverage for doctor's visits, inpatient hospital, ambulance and emergency room care, and more – with Medicare Part D prescription drug coverage. But the plan also helps you stay healthy and get the services you need by offering:

  • Transportation, meal and home care services arrangement
  • Nurse Navigatorsā„  and CareLineā„  when you need assistance after-clinic hours

Friendly help just for you

When you join our MSHO Plan, your personal Care Coordinator will visit you and explain your benefits and how to get the most from the health care system. Your Care Coordinator is a licensed health care professional who can help you improve and/or maintain your health by assisting with prescription drugs and refills; access medical appointments, dental care, transportation, meals, interpreters, and get chore and companion services as well as receive safety modifications to your home as appropriate.
