Through a contract with Medicare, HealthPartners offers these products with Medicare Part D in your area:
- HealthPartners® Freedom
- HealthPartners® Wisconsin Freedom
- HealthPartners® Minnesota Senior Health Options (HMO SNP)
These HealthPartners plans offer Medicare Part D Prescription Drug programs that are optional or embedded in your medical plan so you don't have to enroll in two different plans.
Medicare's "Extra Help" Program
Medicare provides “Extra Help” to pay prescription drug costs for people who have limited income and resources. If you qualify, your monthly plan premium will be lower. View the low income premium subsidy tables for more information:
