Do you like to travel? Do you spend most of the winter in a warmer climate?

With Freedom, you get the Extended Absence Benefit so you can travel outside the plan's service area and within the United States for up to nine months each year and receive covered health care services from non-network providers. Contact Member Services or log on to your account to activate your Extended Absence Benefit. The non-network providers you see must participate in the Medicare program, unless the service is not Medicare covered (such as dental and acupuncture). Some of these providers may require you to pay for services at the time they are provided, but you can then submit a claim to HealthPartners for reimbursement of the services covered by your plan.

Emergency and urgently needed care are covered anywhere in the world under Freedom Vital, Balance and Ultimate. In an emergency, call 911 or go to the nearest hospital or medical emergency center.

The benefit information provided herein is a brief summary, not a comprehensive description of benefits. For more information contact the plan. Benefits, formulary, pharmacy network, premiums and/or copayments may change on January 1, 2013.
