HealthPartners Care Incentive Programs
Making sure our members are healthy is important to HealthPartners. Getting a blood lead test done around age 1 and 2 is an important part of making sure children are healthy. A blood lead test is also a component of a complete Child & Teen Checkup for certain age groups.
Blood Lead Incentive
HealthPartners offers a $25 gift card incentive to HealthPartners Care members who get their 9 to 30-month-old children tested for lead.
Child & Teen Checkup Incentive
HealthPartners offers a $25 gift card incentive to HealthPartners Care members who take their 1 to 5-year-old children for a Child & Teen Checkup. If the child is also due for a blood lead test, you can get that done at the checkup and receive both $25 gift cards.
Printable Voucher
To receive one or both of these gift cards, members can bring this printable voucher to the clinic. A physician or lab technician signature is required to verify that the blood lead test and/or the Child & Teen Checkup is completed.
Targeted Mailing
HealthPartners Care members with children 9 months to 5 years old will also receive mailed information about these incentives. This mailing will include a similar voucher that members can complete and mail back to HealthPartners to receive their gift card(s). A $25 gift card incentive will be sent to eligible members who receive a blood lead test. A physician or lab signature is required.
One gift card per incentive per member. Gift cards will be sent within 4–5 weeks of receipt of signed voucher.
