All new employees are required to complete and turn in new hire paperwork prior to beginning employment and prior to attending orientation.
Please print and complete each of these forms to bring with you to your New Hire Appointment. Note the I-9 form will need to be completed online before your new hire appointment.
Completed new hire paperwork, including:
Proof of your identity and employment eligibility for the I-9 form. Refer to last online page which is the log out page, for list of acceptable unexpired documents. The list of documents varies according to the citizenship status you entered in Section 1 of the I-9. For further information on the I-9 form, click on the I-9 instruction link located throughout the I-9 online system. This documentation is required for employment.
Note: When you complete Section 1 of the online I-9 form you must enter N/A in any fields that you would leave blank. For example, if there is nothing to enter in the fields asking for a middle initial, other last names used, apartment number, email address or phone number, enter N/A. Those fields can no longer be left blank. Also, make sure to use proper capitalization when filling out the I-9 online form.
TIP: Individuals who have not completed and submitted all required paperwork including appropriate forms of ID for I-9 will have their employment start date delayed until the next scheduled orientation session which will be a minimum of a two week delay (four week delay for those in Nursing). Please be sure to complete and turn in all required paperwork on time.
