Summary Measures of Health and Well-Being

Achieving the Triple Aim for population health.

Why measure population health and well-being?

HealthPartners is among health care organizations that collect data on health outcomes. Measuring the quality of care for conditions such as diabetes and depression has made it possible to improve health for patients with specific conditions and to reduce health disparities among different groups of patients.

There is a growing focus on the role of well-being in health. Organizations that measure well-being as an indicator of progress of public health include the Centers for Disease Control and Prevention, Gallup-Sharecare, and the World Health Organization.

To reflect this broader vision of health, HealthPartners updated its mission to include improving health and well-being. To assess progress on achieving our mission, we began developing the Summary Measures of Health and Well-Being in 2014.

What are we measuring?

The Summary Measures of Health and Well-Being address three components:

  1. Current health status
  2. Sustainability of health (lifestyle behaviors and preventive services that determine future health)
  3. Life satisfaction (overall well-being)

Application in strategic planning

The Summary Measures of Health and Well-Being can provide valuable data for strategic planning for health care organizations, schools, policy makers, businesses and other community partners. We hope that the Summary Measures will contribute towards a standard measurement approach to improving health and well-being for individuals and communities.
