Living with a health condition can get in the way of what matters most to you. Pregnancy, asthma and rare health conditions can make tasks that used to be easy much harder. We’re here to help.
What kind of help is available?
Learn how to manage your condition, prevent complications and stay out of the hospital. Eligible members can receive free telephone help from nurses at HealthPartners. Calls are voluntary and confidential.

I received a letter in the mail
If you have certain medical conditions, we may call you or send a letter. We’re inviting you to join one of our condition management programs. The nurse support you may receive varies by condition. The best way to find out what’s available to you is to talk with a nurse. Sign up to have a nurse contact you.
Am I eligible for support from a HealthPartners nurse?
You may be eligible for a condition management program if you have any of the following conditions.
Common eligible conditions

- Coronary artery disease
- Heart failure
- Low back pain
Eligible rare conditions
- Cystic fibrosis
- Gaucher's disease
- Multiple Sclerosis
- Myasthenia gravis
- Parkinson's disease
