We offer four different plans – Sanford Basic, Sanford Vital, Sanford Active and Sanford Ultimate. With a HealthPartners Sanford Medicare plan, you’re getting the best of both companies – Sanford Health’s network of trusted providers, among others, and the Medicare expertise of HealthPartners.

Travel coverage for care when you’re away from home, whether that’s in the United States or across the globe (not available with Basic).

Health club memberships and home fitness kits through the Silver&Fit® Exercise & Healthy Aging Program (not available with Basic).

From Oct. 1 through March 31, we take calls from 8 a.m. to 8 p.m. CT, seven days a week. You’ll speak with a representative. From April 1 to Sept. 30, call us 8 a.m. to 8 p.m. CT, Monday through Friday to speak with a representative. On Saturdays, Sundays and federal holidays, you can leave a message and we’ll get back to you within one business day.

HealthPartners is a health plan that contracts with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. HealthPartners is also a Cost plan and PPO plan with a Medicare contract. Enrollment in HealthPartners depends on contract renewal.
