These terms are in effect as of the date shown above and may have changed since the last time you scheduled an appointment. Please read these terms and conditions of use carefully before confirming your appointment.
By confirming your HealthPartners appointment, you agree that:
- This is not an emergency.
- The information you gave us is correct and can be used to find your medical record. We can also use it to create a new record in our system to schedule your appointment.
- We make every effort to offer you the care you need, when you need it. However, your appointment may not start right at your scheduled time.
- If we have a question or need to change your appointment, we can call you using the phone number you gave us.
- You may get an email confirming your appointment. It can be sent to the email address you gave us. This email will not be encrypted.
Not intended for use during a medical emergency
This Site is not intended for use in a medical emergency or in the case of an urgent health care need. If you have an emergency or urgent health care need, call 911 or seek care as soon as possible.
- Abdominal pain – severe
- Allergic reaction – difficulty breathing or swallowing, severe facial swelling
- Animal bite or human bite
- Assault or abuse
- Bleeding that won’t stop – including nosebleeds
- Breathing problems – including shortness of breath or wheezing that is new or won’t stop
- Burns – excluding sunburn
- Chest pain or chest pressure
- Confusion – lasting confusion that feels different than normal
- Coughing up blood
- Depression – experiencing serious or more serious than usual symptoms
- Dizziness or vertigo
- Electrical shock
- Face swelling, drooping, numbness or asymmetry
- Facial injuries
- Fever – any fever in children under 3 months of age. For other children: 105 degrees or greater. For adults: 103 degrees or greater.
- Headache – unbearable pain or vomiting
- Head injury
- Hearing loss – sudden or unexpected
- Pregnancy – including women in labor, bleeding, water broken, trauma, motor vehicle accidents and lack of fetal movement
- Seizures
- Shoulder or upper arm pain – for new pain that isn’t the result of an injury
- Speech – including slurred speech or difficulty speaking
- Suicidal thoughts or threats
- Throat swelling
- Travel-related sickness or sickness after a recent trip outside the country
- Urination – unable to urinate
- Vision changes – sudden or unexpected
- Vomiting red or black blood
- Weakness or numbness on one side of the body that’s sudden or unexpected
