Our Center for Chronic Care Innovation connects research with practice through development, implementation, dissemination and maintenance of state-of-the-art tools with the potential to improve population health. These include clinical decision support (CDS) and simulated learning tools designed for clinicians and patients.<\/p>\n
We aim to improve the quality of chronic disease care and prevention by promoting research and translating research into innovative practice changes for patients, clinicians and health systems.<\/p>\n
Researchers working with our Center have received more than $50 million in federal funding in the past 15 years to develop and study health delivery interventions. Our research includes a wide range of topics on prevention and care for people with chronic diseases.<\/p>\n
In the past it’s taken a long time to get evidence into practice. Our team has developed and implemented innovative clinical decision support tools for primary care clinicians and patients to use when making decisions about chronic disease care. Our work leverages our integrated medical care delivery system to understand how CDS can improve care delivery and patient outcomes for conditions such as diabetes, hypertension, hyperlipidemia and other cardiovascular risk factors. Our tools translate evidence into practice and are updated to bring evidence-based care into practice efficiently.

Our web-based tools for clinical education mimic an interactive electronic health record and engage clinicians to select care actions for virtual patients over a series of virtual encounters. The simulated education program challenges users to achieve optimal care goals for conditions such as diabetes and hypertension.

We collaborate with other research and health care organizations regionally and nationally to conceptualize, design and implement innovative research strategies to improve chronic disease care and prevention. We also participate in large data studies to advance analytic methods such as machine learning, artificial intelligence, data-mining and other methods that contribute to improving population health.
