If you live in the Twin Cities Metro, SmartCare is a great choice for health care and insurance coverage. It’s a health plan and clinic system working together in a new way. With this plan, you’ll have a network of clinicians to support you, at a lower cost.

SmartCare Network

With the SmartCare plan, you need to use four SmartCare clinics for in-network primary care.  These clinic locations are:

·             HealthPartners SmartCare Clinic - Maplewood

·             HealthPartners SmartCare Clinic – St. Paul

·             Park Nicollet SmartCare Clinic– St. Louis Park

·             Park Nicollet SmartCare Clinic – Burnsville

For specialty and hospital care, you can access a comprehensive network that includes HealthPartners, Park Nicollet, Children’s Minnesota specialty care and select other providers.

There are seven SmartCare plans to choose from. No matter which you choose, you’ll get:

Support from a team of primary care doctors, nurse practitioners, physician assistants and nurses who know you Save money by choosing from different care options. For example, if a member of your care team can provide the care you need over the phone, you pay less than you would for an in-person visit. Convenient ways to get care, including phone, video and in-person visits, with available evening and weekend hours. Prescription insulin for no more than $25 a month
