Minnesota Senior Health Options

HealthPartners Minnesota Senior Health Options (MSHO) (HMO SNP) is designed for seniors on Medical Assistance and Medicare Parts A and B. MSHO offers easy access to primary and specialty health services and your own Care Coordinator to make sure you get the care you need. View complete listing of service areas.

Medical and Prescription Drug coverage – plus more!

The MSHO plan pairs your medical benefits – including coverage for doctor’s visits, inpatient hospital, ambulance and emergency room care, and more – with Medicare Part D prescription drug coverage. But the plan also helps you stay healthy and get the services you need by offering:

Friendly help just for you

When you join our MSHO Plan, your personal Care Coordinator will visit you and explain your benefits and how to get the most from the health care system. Your Care Coordinator is a licensed health care professional who can help you improve and/or maintain your health by assisting with prescription drugs and refills, access medical appointments, dental care, transportation, meals, interpreters, and get chore and companion services as well as receive safety modifications to your home as appropriate.

Learn more about HealthPartners MSHO by reviewing the Summary of Benefits, Annual Notice of Change (ANOC), and Member Handbook (Evidence of Coverage) (EOC).
