Minnesota Health Care Programs

HealthPartners Care plans provide high quality care and services for participants receiving medical assistance through one of the Minnesota Health Care Programs. HealthPartners contracts with the Minnesota Department of Human Services to offer the following plans:

  • HealthPartners Care PMAP is primarily a federally-funded plan for low-income families with children and seniors.
  • HealthPartners Care MinnesotaCare is a state-funded plan for residents who do not have access to affordable health care coverage.
  • HealthPartners Minnesota Senior Health Options (HMO SNP) Plan is specifically designed for people over age 65 who are eligible for Medical Assistance and Medicare Parts A and B. This plan offers easy access to primary and specialty health care with the one benefit that is most comforting: your own Care Coordinator to help coordinate your care.
  • HealthPartners Minnesota Senior Care Plus (MSC+) is a federal and state-funded plan for low-income seniors.
