SmartCare is a new way to do health care

SmartCare gives you access to an enhanced, team-based version of primary care. Our clinic team offers pediatrics, family medicine and internal medicine – including management of chronic diseases, acute symptoms and preventive care.

There are many different ways to receive your care:

  • Traditional office visits
  • Online visits
  • Scheduled telephone visits
  • Video visits

Not all care requires a doctor’s office visit; get options that fit

We’ll work with you on a case-by-case basis to decide which option can best deliver high-quality care at a low cost. Many common health concerns can be resolved through video visits, scheduled phone visits and online visits.

  5. You’re immediately scheduled for a visit – whether it’s in office, online or by phone or video – with the right care team member.

If you’re new to SmartCare, we know you’ve got lots of questions.

How long do I have to wait for an appointment?

SmartCare is all about convenience and flexibility. With multiple care team members and appointment types, we’ll find a time and place that works for you.

Who is on my care team?

The SmartCare team includes doctors, physician assistants, nurse practitioners, registered nurses, licensed practical nurses and medical assistants.

What if I want to see a specific SmartCare team member?

Our team will make this happen; we want to meet your needs.

If I have the SmartCare plan, can I still see the same doctor that I’ve gone to for years?

No, the SmartCare team takes over all your primary care needs. You get an entire team that’s dedicated to keeping you healthy. If you don’t have the SmartCare plan, you can try SmartCare in addition to your current care relationships.

With the SmartCare plan, do I need a referral for specialty and hospital care?

Call your clinic for help accessing a network that includes HealthPartners, Park Nicollet, Children’s Minnesota specialty care and other providers.

With the SmartCare plan, who do I contact with questions about coverage, claims or account balances?

Call the Member Services phone number listed on the back of your ID card.
