Open Access network

Choose from more than 950,000 doctors and 6,000 hospitals in the United States. Simply go to your network doctor when you need care. No need to select a primary care clinic. And you don’t need referrals to see specialists.

The network includes the Mayo Health System, Mayo Clinic–Rochester, St. Mary’s Hospital and Rochester Methodist Hospital.

Open Access Perform network

Choose from more than 950,000 doctors and 6,000 hospitals in the United States. You don’t need to select a primary care clinic, and referrals for specialist aren’t necessary.

The Mayo Health System, Mayo Clinic–Rochester, St. Mary’s Hospital and Rochester Methodist Hospital are not part of this network. But you can still use them with your out-of-network benefits.

NationalONE network

The NationalONE plan ensures you and your family have convenient access to quality health care without out-of-network limits.

The Distinctions network

With Distinctions, you’re in control. All hospitals and clinics are put into three benefit levels. These levels are based on quality and cost. The amount you pay for care depends on which benefit level your hospital or clinic is in.

Select Choice network

With Select Choice, you choose a primary care clinic that meets your needs. Each person in your family can choose a different clinic, and you switch at any time. You have access to 34,000 network doctors and 275 hospitals in Minnesota and Wisconsin. Plus, you don’t need a referral to see a network specialist.

Classic network

With the Classic network, you choose a primary care clinic that meets your needs. Everyone in your family can choose a different clinic, and you can switch at any time. You get access to 33,000 doctors in Minnesota, western Wisconsin, South Dakota and North Dakota.
