This session will share findings from a comprehensive study of health care home transformation in Minnesota, featuring the first 132 clinics to undergo certification. We will discuss findings from two surveys of clinic leaders, one about clinical practice systems and the other about strategies used for transformation. We will share challenges and lessons learned from interviews conducted in 9 diverse clinics, results of an analysis of healthcare cost data, and insights about our process for integrating patients and clinicians into research study operations.
