This recent publication advocates modifications in how clinical guidelines are constructed to (a) prioritize recommendations based on magnitude of clinical impact, (b) prioritize recommendations based on costs, and (c) use these prioritized recommendation lists to negotiate with patients and accommodate patient preferences within a rational evidence-based context. It is clear that the electronic medical record (EMR) can be a tool to advance these agendas.
