Provider credentialing and enrollment

Credentialing is the process of evaluating and verifying a practitioner’s qualifications to participate as a provider with HealthPartners.

HealthPartners doesn’t credential certain types of practitioners. Instead, these practitioners are enrolled with us to bill and receive reimbursement.

HealthPartners credentials contracted health care facilities and vendors to ensure compliance with our established quality and care delivery standards.

Manage your locations and practitioners with Provider Data Profiles

Contracted provider groups can make additions and updates to location and practitioner listings using the online Provider Data Profiles application. This tool allows you to review, add and update specific profile information:

Locations – addresses, appointment phone numbers, services, hours and more
Practitioners – credentialed information, specialties, affiliated clinics and hospitals, personal profiles and more

If you don’t have a provider portal user account or use the Provider Data Profiles application, contact your organization's delegate.

Once you’ve joined our network as a contracted provider or been offered a contract with HealthPartners, then you can get credentialed.

Use the Provider Data Profiles application to make additions and changes, and to create a shareable PDF:

Medical, behavioral health and MTM practitioners should submit the Minnesota Uniform Practitioner Change form (PDF) with the Site Location Addendum page.
Dental providers should complete the Dental Contracted Provider Change Notice (PDF)

Use the Add Practitioner search in the Provider Data Profiles application to find or enroll practitioners.

If they’re not in the Provider Data Profiles, use the Minnesota Uniform Practitioner Change form (PDF), with the Site Location Addendum page if applicable), and fax it to 952-883-5160 or email it to ProviderData@HealthPartners.com.

The process can vary depending on whether the application is complete, whether we need more information from the practitioner and whether the practitioner meets all the HealthPartners credentialing criteria.

We’ll notify practitioners in writing once their application has been approved. You can check if a practitioner has been approved:

To maintain compliance with the HealthPartners network and credentialing requirements, all enrolled and credentialed providers must notify us whenever a practitioner leaves a contracted clinic.
