What is the CMS TEAM program?

The CMS Transforming Episode Accountability Model (TEAM) is a new initiative from the Centers for Medicare & Medicaid Services (CMS) designed to improve care for patients enrolled in traditional Medicare (Parts A & B) who undergo certain surgical procedures at qualifying hospitals. The HealthPartners hospital participants are Lakeview Hospital, Methodist Hospital and Regions Hospital. We are participating in this new program to help ensure high-quality, well-coordinated care.

The program covers five types of surgical procedures:

  • Coronary artery bypass graft
  • Lower extremity joint replacement
  • Small and large bowel procedures
  • Spinal fusion
  • Surgical hip femur fracture treatment

Who are the TEAM collaborators?

TEAM collaborators are skilled nursing facilities, home health agencies and physician groups who work together to support your care.
