As a patient, you or your legal representative may inspect or obtain a copy of your medical records, request a change, or have copies of medical records sent to another person or facility.
To fulfill these requests, we require a completed and signed written request or authorization for the release of health information. We ask that you let us know what components of your medical records you want to obtain. The discharge summary, operative report, and history and physical often contain the relevant information to suit your needs.
You can use any of the following methods to request your medical records:
- Sign in to your HealthPartners online account and select My Care, then Health Records, then Get Documents and Records.
- Complete our online medical record request form
- Submit a completed, printed Authorization for Release of Information (PDF) form to your clinic.
If you’d like your care team to verbally share information with family or friends, please complete a Permission to Verbally Discuss Protected Health Information (PDF) form and submit it to your clinic.
If you’d like to request a change to your medical records, please use a Request for Correction/Amendment of Health Information (PDF) form.
