These terms are in effect as of the date shown above and may have changed since the last time you scheduled an appointment. Please read these terms and conditions of use carefully before confirming your appointment.

By confirming your HealthPartners appointment, you agree that:

  • This is not an emergency.
  • The information you gave us is correct and can be used to find your medical record. We can also use it to create a new record in our system to schedule your appointment.
  • We make every effort to offer you the care you need, when you need it. However, your appointment may not start right at your scheduled time.
  • If we have a question or need to change your appointment, we can call you using the phone number you gave us.
  • You may get an email confirming your appointment. It can be sent to the email address you gave us. This email will not be encrypted.
  • If you include your mobile phone number in your online account, you give us permission to send you text messages that relate to your relationship with HealthPartners. You can also sign up to get appointment reminders and important updates about your health care via text at HealthPartners clinics, including appointment scheduling confirmations, reminders or cancelations. Not intended for use during a medical emergency

This Site is not intended for use in a medical emergency or in the case of an urgent health care need. If you have an emergency or urgent health care need, call 911 or seek care as soon as possible.
