Home care helps you manage your health or recover from illness, injury or surgery in the comfort of your home. It’s designed for people who are homebound and need short-term medical care where they live.
At HealthPartners and Park Nicollet, we offer support designed to meet your needs or the needs of someone you care about. For more than 30 years, people have turned to us for medical, nursing, social or therapeutic treatment wherever they live, whether that’s a house, apartment building, group home or assisted living facility.
Our board-certified doctors direct more than 65,000 home visits and related services each year throughout our service area in Minnesota and western Wisconsin.
We work with our team across 50+ specialties to help make sure you get the care you need at home, saving you clinic visits. You’ll also have an on-call nurse who can help with medical concerns 24 hours a day.
Home care is health care you receive in your home. The purpose is to help you manage your health or recover from illness or injury when traveling to receive care is not an option.
Many people confuse home care with personal care assistance. Home care is rehabilitation and skilled nursing services that a doctor orders.
Personal care assistance typically includes help around the house and light meal prep. Sometimes people who need home care also need personal care assistance. Medicare covers home care 100%. Personal care assistance is usually paid out of pocket, and it is not offered by HealthPartners and Park Nicollet.

A nurse or therapist will usually come to your home within 48 hours of your application to discuss your needs. They’ll begin by asking about your medical needs and other support and personal assistance that may be required.
We are a Medicare- and Medicaid-certified and state-licensed agency. If you or someone you care for meets the eligibility requirements and the home care program is approved by Medicare or Medicaid, private insurance, Medicare or Medicaid should cover the following services for a defined period of time:
