We also offer financial assistance for qualifying patients who are having trouble with their medical bills. Help may be available through the HealthPartners Clinics Financial Assistance Program, Regions Hospital Financial Assistance Program or the Minnesota Health Care Program.

We can give you a good faith estimate of the cost for future services at HealthPartners Clinics and Regions Hospital. If you’re a HealthPartners member, we’ll work with Member Services to provide an estimate of fees not paid by your HealthPartners insurance.

An e-statement is identical to a paper statement, but it can be viewed, printed or saved right from your computer. Sign up for e-statements online through your online account. You’ll need the myEasyMatch® code from your paper statement and a valid email address to enroll.

After you enroll online, you’ll receive a validation email. Click on the link in the email to activate your online account. Once you sign up for e-statements, you’ll stop receiving paper statements.

Wondering how much you can expect to pay? Call our financial counselors at 952-993-5586 for a good-faith estimate of the cost of care. We base the estimate on information you provide. Please be aware that actual charges may be greater or less than the estimate if the level of care or the services you receive end up being different.

We aren’t able to tell you if the requested service is covered by your individual insurance plan. It’s best to call your insurance company to confirm coverage.

We accept most major insurance plans. We recommend that you contact your insurance company before scheduling an appointment. Your insurance company can tell you which services are covered and whether the clinic or hospital you plan to visit is in-network. If your insurance card lists a primary care provider other than Park Nicollet Clinic or Park Nicollet Methodist Hospital, you may need a referral from your insurance company to be seen at Park Nicollet.

Park Nicollet no longer accepts Medicare Advantage private, fee-for-service plans.

If you need us to send your medical record information to a non-Park Nicollet provider, please complete the Patient Authorization for Release of Protected Health Information.

If you need medical record information sent to us from a non-Park Nicollet hospital or clinic, please contact that facility.

If you have questions or need help with your medical records, refer to the information sheet or call Health Information Management at 952-993-7600. If you have questions regarding radiology images, call 952-993-5402.

At Park Nicollet, we understand the importance of privacy when it comes to your health information – that's why we don't discuss your medical care with anyone that you haven't specifically authorized. If you'd like certain friend, family members or others to be involved in your care, please complete the Permission to Verbally Discuss Protected Health Information with Friends and Family. This will let us know who you’d like us to talk to regarding appointment information, lab and test results, treatment information and other medical information that you authorize.
