Member outreach initiatives

About Member Outreach

Our member outreach program delivers customized information directly to members about their needed preventive and chronic care services. At least annually, preventive and chronic care reminders are sent to members due for specific services based on claims data. These reminders are in the form of letters and Secured Web Mail (SWM) messaging technology. Experience has demonstrated that personalized letters vs. postcards or recorded calls are the most effective and preferred communication method.

Your clinical care is supported by the member outreach program by offering general education and decision-making tools for the identified services. Patients are directed to contact Member Services for information on benefits and coverage.

Preventive service outreach:
  • Colon cancer screening
  • Breast cancer screening
  • Cervical cancer screening
  • Pediatric Immunizations
Chronic care outreach:
  • Diabetic LDL and A1C screening
  • Cardiovascular LDL screen
  • Diabetic Eye Exam
  • COPD & Spirometry testing (reminders sent monthly)
