SmartCare insurance plan and clinics

SmartCare – the clinic and plan that revolve around you

Whether you enroll in SmartCare through an insurance plan or sign up directly with our clinics, our team strives to save you time and money. You’ll get the high-quality care you expect, redesigned to better fit your busy lifestyle.

SmartCare is a new way to do health care

SmartCare gives you access to an enhanced, team-based version of primary care. Our clinic team offers pediatrics, family medicine and internal medicine – including management of chronic diseases, acute symptoms and preventive care.

There are many different ways to receive your care:

  • Traditional office visits
  • Online visits
  • Scheduled telephone visits
  • Video visits

Not all care requires a doctor’s office visit; get options that fit

We’ll work with you on a case-by-case basis to decide which option can best deliver high-quality care at a low cost. Many common health concerns can be resolved through video visits, scheduled phone visits and online visits.

Here’s how to schedule your next visit

  1. Schedule online at or call the clinic where you’d prefer to go for in-office visits:
952-967-5520 for Maplewood and St. Paul
952-967-5584 for Maplewood and St. Paul
952-993-1190 for Burnsville and St. Louis Park
  2. Tell us your symptoms or the reason for your visit.
  3. We’ll let you know which team members can best meet your needs – and what visit types are available.
  4. You’ll receive upfront cost so you can make an informed decision.
  5. You’re immediately scheduled for a visit – whether it’s in office, online or by phone or video – with the right care team member.
