Once you’re enrolled in a HealthPartners MSHO plan, reward dollars are automatically added to a prepaid MasterCard after you complete eligible routine visits and screenings. You’ll get your rewards card in the mail automatically. You don’t need to do anything extra.

2026 MSHO rewards you can earn

Stay healthy and earn rewards when you complete important preventive care. These visits won’t cost you anything if you go to an in-network care provider.

For screenings, your in-network provider needs to ensure the screening is appropriate for you. If it is, you must be enrolled in an MSHO plan when the provider completes the screening to earn the reward.

Under your MSHO coverage, there are two types of preventive care visits you can earn rewards for:

  1. The first is your Welcome to Medicare Visit. This is an initial preventive care visit for new-to-Medicare patients who have been members for less than one year. If it’s your first year on your HealthPartners MSHO plan, and it’s also the first year you’ve ever had Medicare coverage, you’ll earn $100 when you complete this visit.
  2. The second is your Medicare Annual Wellness Visit. This is a yearly visit where you speak with your doctor about your health and well-being. You can schedule this visit once each year after your first year with Medicare coverage. You’ll earn $100 every time you complete this visit.

A mammogram is an essential health screening for women. Yearly mammograms are recommended to screen for cancer or any other abnormalities in your breasts.

If you don’t have a history of breast cancer or mastectomy, your MSHO plan covers one preventive breast cancer screening per year. Once you complete this screening, you’ll earn $100.

Bone density naturally decreases with age, and there aren’t usually any advance symptoms of related conditions. That’s why screenings are crucial. Your doctor can track your bone health over time to help you stay strong.

Your MSHO plan covers one preventive bone density screening per year. Screenings are covered for women starting at age 65. Screenings are covered for men typically starting at age 70.

Once you complete this screening, you’ll earn $100. (Members living in hospice or skilled nursing facilities are not eligible.)

Colorectal cancer often begins with no symptoms. That’s why regular colorectal screening is so important for both men and women. If cancer is caught early, it’s more easily treatable.

Your MSHO plan covers preventive stool tests (including at-home stool tests), colonoscopies and sigmoidoscopies once every two years for high-risk individuals, and once every 10 years for low-risk individuals. When your screening is completed, you’ll earn $100.

An A1c screening tests blood sugar to help people manage diabetes.

If you have Type 1 or Type 2 diabetes, you can earn a $50 reward once per year after you complete an A1c screening at an in-network provider.

How HealthPartners MSHO reward cards work

When you earn your first reward, you’ll get a welcome letter and your reward card in the mail. Use your reward card to shop at select stores near where you live for groceries, personal care items and more. A complete list of eligible stores is below.

All future rewards you earn will be loaded onto the same card. Rewards are loaded automatically within five weeks after your doctor sends us a claim following an eligible screening. Every time you earn a reward, you’ll get a letter telling you how much you earned.

Keep your card and don’t throw it away, even if the balance reaches $0. It’s reusable and reloadable for up to five years. Before it expires, we’ll automatically send you a new card in the mail.

If you have a HealthPartners MSHO plan, you can earn rewards.

Keep in mind that in order to earn rewards, an in-network provider needs to confirm that a screening is medically necessary for you. If they do, you need to already be enrolled in MSHO when your provider completes the screening to earn the reward.

You can earn a reward for completing your Medicare Annual Wellness Visit once per year.

If your in-network provider thinks these screenings are right for you, you can also earn a reward for:

  • A breast cancer screening, once per year
  • An A1c screening at an in-network provider, once per year
  • A bone density screening, once per year
  • A colorectal cancer screening. You can earn this reward once per year, but your plan only covers screening once every two years for high-risk individuals or once every 10 years for low-risk individuals.

You can only earn a reward for completing your Welcome to Medicare Visit once, and only if it’s your first year on our MSHO plan and the first year you’ve ever had Medicare coverage. You can’t earn this reward and the Medical Annual Wellness Visit reward in the same calendar year.

Simply make eligible appointments and complete them. There’s nothing extra you need to do.

Once we get confirmation from your doctor through our normal claims process that you’ve completed an eligible screening, we’ll load your reward onto your rewards card within five weeks.

You can use your rewards online or in person at participating stores from these companies:

  • Aldi
  • Cash Wise
  • Coborn’s
  • Cub
  • CVS
  • Fresh Thyme
  • Hy-Vee
  • Target
  • Trader Joe’s
  • Walgreens
  • Walmart

You can use your rewards card at participating stores for groceries, clothing, personal care items and much more. But you can’t use it to buy gift cards, tobacco, alcohol or firearms.

No. When you get the card in the mail, it’s already activated. All you need to do is sign the back.

No, but you can if you want to.

  • Without a PIN number, choose “credit” when using your card.
  • If you’d like to set up a PIN number, first call the number on the back of your card. Then, choose “debit” when using your card and enter the PIN number you chose.

You can sign in to your HealthPartners account. Look for the trophy icon at the top of the page.

You can also check your balance by contacting Member Services.

Yes. Any rewards earned in a calendar year must be used by June 30 of the following year. For example, rewards earned in 2026 need to be used by June 30, 2027. Otherwise, they’ll expire.

Your card could be declined for several reasons, including if:

  • The total cost of the transaction is more than the funds available on your card
  • You aren’t using your card at a participating store
  • The location doesn’t accept your Mastercard rewards card
  • You’re trying to buy a prohibited item
  • Your reward dollars have expired

No. Rewards are an additional perk available to you at no extra cost as a HealthPartners MSHO member.

If your card is lost or stolen, contact Member Services.

We’ll close your card and send you a new one. Plus, if you think your reward card was used fraudulently, you can dispute the charge.

The first step is to contact Member Services.

We’ll collect information from you like the date of your visit, which doctor you saw and the screening you received. Then we’ll work to determine your eligibility for the reward.

The rewards process usually takes several weeks. Contact Member Services if you have questions about your rewards status.
