Uno Health works alongside your HealthPartners Medicare Advantage plan to help you access programs that can save you money. Members often save on groceries, utility bills, medical expenses and more.
Uno Health provides start-to-finish support by finding programs you’re eligible for, completing applications and submitting paperwork for you. They can even help with annual re-enrollment in these assistance programs, if needed.
This support is available to all HealthPartners Medicare Advantage members at no extra cost.
Assistance programs require a lot of time and paperwork. It can be tough to know what’s out there, where to apply or if you even qualify. That’s why Uno Health does all the work. They find, screen and apply to programs for you. You’ll get one-on-one help from enrollment specialists who will handle your paperwork. They’ll ensure you’re not missing out on any benefits. They make the process simple.
HealthPartners works with Uno Health to support your health and well-being so you can get everything you’re eligible for. Here’s what HealthPartners Medicare Advantage members can expect:
- You’ll get an email from Uno Health with a link to complete some questions that help them see which programs you may qualify for. If you’d like to answer the questions over the phone, there’s a dedicated phone number you can call.
- If Uno Health determines you’re eligible for programs, they’ll let you know which ones and which documents they need to enroll you so you can start saving.
- Once you’ve submitted your documents, Uno Health goes to work for you. They’ll complete applications and follow up on your behalf. They’ll let you know when you’re approved, and you’ll find out your expected savings in a few weeks.
Remember: As you work with Uno Health, they’ll always ask you to confirm your name and address whenever you talk on the phone. This is to protect your privacy and ensure they’re speaking with the right person. Many Medicare members don’t realize they’re eligible for financial assistance programs. From savings on heating bills to groceries and more, Uno Health can help you figure out what you qualify for. For example:
- Medicare Savings Programs (MSPs) can pay all or part of your Part B premiums and any Part A premiums you might be responsible for. They can also cover copays, deductibles and coinsurance.
- The Low-Income Subsidy/Extra Help program ensures qualifying members pay low or no costs for Medicare Part D (prescription drug coverage).
- Medical Assistance (also called Medicaid) provides comprehensive health insurance to low-income individuals. If you qualify, you can complement your Medicare coverage and reduce your health care costs significantly. You can also combine Medical Assistance and Medicare with a Minnesota Senior Health Options (MSHO) plan.
- The Supplemental Nutrition Assistance Program (SNAP) provides food and grocery funds each month for low-income individuals and families. This program is also known as FoodShare in Wisconsin.
- The Low-Income Home Energy Assistance Program (LIHEAP) is a seasonal program that provides assistance for heating bills. It can also provide additional support in emergencies.
- The Lifeline program makes communication more affordable by providing a monthly discount on phone or internet services to qualifying individuals and families.
- Supplemental Security Income (SSI) provides monthly payments to people 65 and older who have limited resources. It’s designed to help cover basic needs and expenses.
- Supportive Housing provides affordable rental housing with supportive services. And the Housing Choice Voucher program can help you pay rent.
