Accountable Care Organization

Park Nicollet and HealthPartners Medical Group participates as a Direct Contracting Entity (DCE) in the Global Professional Direct Contracting (GPDC) Model. A DCE is a group of doctors, hospitals and other health care providers who agree to work together to keep you healthy. All members of Park Nicollet Health Services Accountable Care LLC agree to work together to give their patients better quality care. We transitioned from the Park Nicollet Next Generation Accountable Care Organization (NGACO) at the end of 2021.

For general questions or additional information about Direct Contracting Entities, please visit medicare.gov/acos.html or call 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486-2048.
