Once you’re enrolled in a HealthPartners MSHO plan, reward dollars are automatically added to a prepaid MasterCard after you complete eligible routine visits and screenings. You’ll get your rewards card in the mail automatically. You don’t need to do anything extra.

MSHO rewards you can earn

Stay healthy and earn rewards when you complete important preventive care. These visits won’t cost you anything if you go to an in-network care provider.

For screenings, your in-network provider needs to ensure the screening is appropriate for you. If it is, you must be enrolled in an MSHO plan when the provider completes the screening to earn the reward.

How HealthPartners MSHO reward cards work

When you earn your first reward, you’ll get a welcome letter in the mail including your reward card. Use your reward card to shop at select stores near where you live for groceries, personal care items and more. A complete list of eligible stores is below.

All future rewards you earn will be loaded onto the same card. Rewards are loaded automatically within five weeks after your doctor sends us a claim following an eligible screening. Every time you earn a reward, you’ll get a letter telling you how much you earned.

Keep your card and don’t throw it away, even if the balance reaches $0. It’s reusable and reloadable for up to five years. Before it expires, we’ll automatically send you a new card in the mail.
